Neurodegenerative Parkinson's Disease is one of the common incurable diseases among the elderly. Clinical assessments are characterized as standardized means for PD diagnosis. However, relying on medical evaluation of a patient's status can be subjective to physicians' experience, making the assessment process susceptible to human errors. The use of ICT-based tools for capturing the status of patients with PD can provide more objective and quantitative metrics. In this vein, the Personalized Serious Game Suite and intelligent Motor Assessment Tests, produced within (...) the i-PROGNOSIS European project, are explored in the current study. More specifically, data from 27 patients with PD at Stage 1 and Stage 3 produced from their interaction with PGS/iMAT are analyzed. Five feature vector scenarios are set, including features from PGS or iMAT scores or their combination, after also taking into consideration the age of patients with PD. These FVs are fed into three machine learning classifiers, i.e., K-Nearest Neighbor, Support Vector Machines, and Random Forest, to infer the stage of each patient with PD. A Leave-One-Out Cross-Validation method is adopted for testing the classification performance. The experimental results show that a high classification accuracy is achieved from both data sources, justifying the effectiveness of PGS/iMAT to efficiently reflect the motor skill status of patients with PD and further potentiating PGS/iMAT enhancement with a machine learning a part to infer for the stage of patients with PD. Clearly, this integrated approach provides new opportunities for remote monitoring of the stage of patients with PD, contributing to a more efficient organization and set up of personalized interventions. Electroencephalographic monitoring provides critical diagnostic and management information about patients with epilepsy and seizure mimics. Admission to an epilepsy monitoring unit (EMU) is the gold standard for such monitoring in major medical facilities worldwide. In many countries, access can be challenged by limited resources compared to need. Today, triaging admission to such units is generally approached by unwritten protocols that vary by institution. In the absence of explicit guidance, decisions can be ethically taxing and are easy to challenge. In an (...) effort to address this gap, we propose a two-component approach to EMU triage that takes into account the unique landscape of epilepsy monitoring informed by triage literature from other areas of medicine. Through the strategic component, we focus on the EMU wait list management infrastructure at the institutional level. Through the principled component, we apply a combination of the ethical principles of prioritarianism, utilitarianism and justice to triage; and we use individual case examples to illustrate how they apply. The effective implementation of this approach to specific epilepsy centres will need to be customised to the nuances of different settings, including diverse practice patterns, patient populations and constraints on resource distribution, but the conceptual consolidation of its components can alleviate some of the pressures imposed by the complex decisions involved in EMU triage. ABSTRACTRemembering to perform a behaviour in the future, prospective memory, is essential to ensuring that people fulfil their intentions. Prospective memory involves committing to memory a cue to action, and later recognising and acting upon the cue in the environment. Prospective memory performance is believed to be influenced by the emotionality of the cues, however the literature is fragmented and inconsistent. We conducted a systematic search to synthesise research on the influence of emotion on prospective memory. Sixty-seven effect sizes were (...) extracted from 17 articles and hypothesised effects tested using three meta-analyses. Overall, prospective memory was enhanced when positively-valenced rather than neutral cues were presented. In contrast, negatively-valenced cues did not enhance prospective memory overall, but this effect was moderated by the timing of the emotional manipulation. Objective To evaluate the modes of death and treatment offered in the last 24 h of life to patients dying in 10 Brazilian intensive care units (ICUs) over a period of 2 years. Design and setting Cross-sectional, multicentre, retrospective study based on medical chart review. The medical records of all patients that died in seven paediatric and three adult ICUs belonging to university and tertiary hospitals over a period of 2 years were included. Deaths in the first 24 h of (...) admission to the ICU and brain death were excluded. Intervention Two intensive care fellows of each ICU were trained in fulfilling a standard protocol (κ=0.9) to record demographic data and all medical management provided in the last 48 h of life. The Student t test, Mann–Whitney U test, χ2 test and RR were used for data comparison. Measurements and main results 1053 medical charts were included (59.4% adult patients). Life support limitation was more frequent in the adult group (86% vs 43.5%; p<0.001). A ‘do not resuscitate’ order was the most common life support limitation in both groups (75% and 66%), whereas withholding/withdrawing were more frequent in the paediatric group (33.9% vs 24.9%; p=0.02). The life support limitation was rarely reported in the medical chart in both groups (52.6% and 33.7%) with scarce family involvement in the decision making process (23.0% vs 8.7%; p<0.001). Conclusion Life support limitation decision making in Brazilian ICUs is predominantly centred on the medical perspective with scarce participation of the family, and consequently several non-coherent medical interventions are observed in patients with life support limitation. Human-Computer Interaction and games set a new domain in understanding people’s motivations in gaming, behavioral implications of game play, game adaptation to player preferences and needs for increased engaging experiences in the context of HCI serious games. When the latter relate with people’s health status, they can become a part of their daily life as assistive health status monitoring/enhancement systems. Co-designing HCI-SGs can be seen as a combination of art and science that involves a meticulous collaborative process. The design elements (...) in assistive HCI-SGs for Parkinson’s Disease patients, in particular, are explored in the present work. Within this context, the Game-Based Learning design framework is adopted here and its main game-design parameters are explored for the Exergames, Dietarygames, Emotional games, Handwriting games, and Voice games design, drawn from the PD-related i-PROGNOSIS Personalized Game Suite holistic approach. Two main data sources were involved in the study. In particular, the first one includes qualitative data from semi-structured interviews, involving 10 PD patients and four clinicians in the co-creation process of the game design, whereas the second one relates with data from an online questionnaire addressed by 104 participants spanning the whole related spectrum, i.e., PD patients, physicians, software/game developers. Linear regression analysis was employed to identify an adapted GBL framework with the most significant game-design parameters, which efficiently predict the transferability of the PGS beneficial effect to real-life, addressing functional PD symptoms. The findings of this work can assist HCI-SG designers for designing PD-related HCI-SGs, as the most significant game-design factors were identified, in terms of adding value to the role of HCI-SGs in increasing PD patients’ quality of life, optimizing the interaction with personalized HCI-SGs and, hence, fostering a collaborative human-computer symbiosis. Environmental factors routinely influence an organism's biology. The inheritance or transmission of such influences to descendant generations would be an efficient mode of information transfer across generations. The developmental stage at which a specific environment is encountered by the ancestral generation, and the number of generations over which information about that environment is registered, determines an inter‐ vs. trans‐generational effect of ancestral influence. This commentary will outline the distinction between these influences. While seductive in principle, inter‐ and trans‐generational inheritance in (...) mammals is a hotly debated area of research inquiry. We present constructive criticism of such inheritance, and suggest potential experimental avenues for reconciliation. Finally, epigenetic mechanisms present an avenue for gene regulation that is dynamic. We briefly discuss how such malleability affords the potential for a reversal of any detrimental environmental influences that might have adversely impacted ancestral or descendant generations. CYBATHLON is an international championship where people with severe physical disabilities compete with the aid of state-of-the-art assistive technology. In one of the disciplines, the BCI Race, tetraplegic pilots compete in a computer game race by controlling an avatar with a brain-computer interface. This competition offers a perfect opportunity for BCI researchers to study long-term training effects in potential end-users, and to evaluate BCI performance in a realistic environment. In this work, we describe the BCI system designed by the team (...) Mirage91 for participation in the CYBATHLON BCI Series 2019, as well as in the CYBATHLON 2020 Global Edition. Furthermore, we present the BCI’s interface with the game and the main methodological strategies, along with a detailed evaluation of its performance over the course of the training period, which lasted 14 months. The developed system was a 4-class BCI relying on task-specific modulations of brain rhythms. We implemented inter-session transfer learning to reduce calibration time, and to reinforce the stability of the brain patterns. Additionally, in order to compensate for potential intra-session shifts in the features’ distribution, normalization parameters were continuously adapted in an unsupervised fashion. Across the aforementioned 14 months, we recorded 26 game-based training sessions. Between the first eight sessions, and the final eight sessions leading up to the CYBATHLON 2020 Global Edition, the runtimes significantly improved from 255 ± 23 s to 225 ± 22 s, respectively. Moreover, we observed a significant increase in the classifier’s accuracy from 46 to 53%, driven by more distinguishable brain patterns. Compared to conventional single session, non-adaptive BCIs, the inter-session transfer learning and unsupervised intra-session adaptation techniques significantly improved the performance. This long-term study demonstrates that regular training helped the pilot to significantly increase the distance between task-specific patterns, which resulted in an improvement of performance, both with respect to class separability in the calibration data, and with respect to the game. Furthermore, it shows that our methodological approaches were beneficial in transferring the performance across sessions, and most importantly to the CYBATHLON competitions. (shrink)

The coronavirus disease pandemic fundamentally disrupted humans’ social life and behavior. Public health measures may have inadvertently impacted how people care for each other. This study investigated prosocial behavior, its association well-being, and predictors of prosocial behavior during the first COVID-19 pandemic lockdown and sought to understand whether region-specific differences exist. Participants from eight regions clustering multiple countries around the world responded to a cross-sectional online-survey investigating the psychological consequences of the first upsurge of lockdowns in spring 2020. Prosocial behavior (...) was reported to occur frequently. Multiple regression analyses showed that prosocial behavior was associated with better well-being consistently across regions. With regard to predictors of prosocial behavior, high levels of perceived social support were most strongly associated with prosocial behavior, followed by high levels of perceived stress, positive affect and psychological flexibility. Sociodemographic and psychosocial predictors of prosocial behavior were similar across regions. (shrink)

Establishing the basic knowledge, methodology, and technology for a framework for the continuous decoding of hand/arm movement intention was the aim of the ERC-funded project “Feel Your Reach”. In this work, we review the studies and methods we performed and implemented in the last 6 years, which build the basis for enabling severely paralyzed people to non-invasively control a robotic arm in real-time from electroencephalogram. In detail, we investigated goal-directed movement detection, decoding of executed and attempted movement trajectories, grasping correlates, (...) error processing, and kinesthetic feedback. Although we have tested some of our approaches already with the target populations, we still need to transfer the “Feel Your Reach” framework to people with cervical spinal cord injury and evaluate the decoders’ performance while participants attempt to perform upper-limb movements. While on the one hand, we made major progress towards this ambitious goal, we also critically discuss current limitations. (shrink)

Apresentam-se os resultados clínicos principais de uma primeira investigação efectuada a familiares de doentes sob tratamento paliativo no domicílio da área de Lisboa, com o instrumento de medida da satisfação SERVQUAL Modificado. Dos 58 familiares/doentes que responderam ao questionário apenas uma minona estava insatisfeita (uma classe de 5 indivíduos mostra-se francamente insatisfeita), uma classe de 15 estava moderadamente satisfeita, havendo 38 individuos fortemente satisfeitos com a qualidade e prontidäo dos serviços prestados. Porém urna percentagem elevada de doentes, segundo a opinião (...) retrospectiva dos cuidadores, faleceu com sintomatologia agravada, nomeadamente dores (60%), obstipação (67%) e depressão/ansiedade (43%). Discute-se a consistência e validade do questionáno SERVQUAL Modificado, contextualizam-se os resultados no que é o standard da prática da MCP e integram-se na promoção da dignidade humana no fim da vida. We present the main clinical findings of research on palliative home care, carried out among relatives of patients in the Lisbon area. An adapted version of SERVQUAL was used. Of a total of 58 carers surveyed only a small number (5) was unsatisfied 15 were only moderately satisfied. 38 family carers were strongly satisfied with the services. Yet, a high percentage of patients, as viewed in retrospect by their relatives died with some aggravated symptoms: 60% with more pain, 67% constipation, and 43% anxiety/depression. We discuss the consistency and validity of this adapted SERVQUAL tool and the main results are contextualized beanng in mind the standard practice of palliative medicine and the pursuit of human dignity in death. O presente texto parte de um diálogo com um artigo publicado recentemente na revista HOPOS (Journal of the International Society for the History of Philosophy of Science), de autoria dos pesquisadores Fons Dewulf e Massimiliano Simons, intitulado “Positivism in Action: The case of Louis Rougier” (2020). No artigo, os pesquisadores abordam a trajetória pouco frequentada de Louis Rougier, filósofo francês cuja fortuna crítica associou-se à sua participação na rede de intelectuais liberais do pós-guerra e a sua função central na organização (...) do Colóquio Walter Lippmann, ainda em 1938. Os autores abordam a trajetória de Rougier de maneira disciplinarmente inusual: procuram interpretar suas teorias filosóficas à luz de seus comprometimentos políticos, defendendo uma imbricação entre ambas as facetas do pensamento de Rougier. Quero sugerir que uma abordagem disposta a compreender as relações dinâmicas entre uma trajetória intelectual e seu entorno social se beneficia de uma abordagem como essa, às quais pode ser acrescida uma dimensão propriamente institucional das trajetórias intelectuais. Para observá-la, proponho acionar o debate sobre virtudes epistêmicas tal como tem sido pensado por autores como Herman Paul, Lorraine Daston e J. Dongen, com o objetivo de investigar se o debate sobre as non-textual things da atividade intelectual podem ser um caminho para transitar de forma profícua entre texto e contexto. This study investigated the performance of boys with psychopathic tendencies and comparison boys, aged 9 to 17 years, on two tasks believed to be sensitive to amygdala and orbitofrontal cortex func- tioning. Fifty-one boys were divided into two groups according to the Psychopathy Screening Device (PSD, P. J. Frick & R. D. Hare, in press) and presented with two tasks. The tasks were the gambling task (A. Bechara, A. R. Damasio, H. Damasio, & S. W. Anderson, 1994) and the Intradimensional/ (...) Extradimensional (ID/ED) shift task (R. Dias, T. W. Robbins, & A. C. Roberts, 1996). The boys with psychopathic tendencies showed impaired performance on the gambling task. However, there were no group differences on the ID/ED task either for response reversal or extradimensional set shifting. The implications of these results for models of psychopathy are discussed. Heidegger’s recently published 1932 seminar on Plato’s Phaedrus arguably represents his most successful dialogue with Plato, where such dialogue is characterized by both the deepest affinity and the most incisive opposition. The central thesis of Heidegger’s interpretation is that the Phaedrus is not simply a logos about eros, but rather an attempt to show that eros is the very essence of logos and that logos is thereby in its very essence dia-logue. Heidegger is thus here more attuned than ever before (...) to the erotic and dialogical character of philosophy while at the same time concerned with how this conception of philosophy can lead to the eclipse of being and truth. La organización corría a cargo de la renovada Österreichische Ludwig Wittgenstein Geellschaft, que preside Rudolf Haller y cuenta con Jaakko Hintikka como principal scholar. El Congreso ha servido para reunir un buen número de especialistas en esta materia, entre los que figuran M. Dummett, H. Field, I. Grattan-Guinness, P. Maddy, Ch. Parsons, St. Shapiro (...) y H. Wang. Respecto del proyecto inicial sólo hubo una ausencia relevante: el profesor Crispin Wright, conocido especialmente en esta disciplina por su libro sobre la concepción fregeana del número como objeto y su estudio acerca del pensamiento wittgensteiniano sobre los fundamentos de la Matemática. Unethical employee behavior (UEB), an important organizational phenomenon, is dynamic and multi-faceted. Recent renewed interest in the role of emotion in ethical decision-making (EDM) suggests that unethical behaviors are neither always rationally derived nor deliberately undertaken. This study explores how to integrate the conscious and nonconscious dimensions of unethical decision-making. By broadening the scope of inquiry, we explore how integral affect—the emotion tied to anticipated decision outcomes for the employee engaging in misconduct—can shed light on UEB. We review related literature (...) on affect and EDM and propose a model in which we assert that at a decision point, an employee experiences multiple integral affects that are either persuasive or dissuasive toward UEB. We further posit that among these integral affects, a dominant integral affect (DIA) emerges, determining the nature and direction of unethical behavior. In addition, our model considers the role of affective residue and the influence of other factors, such as incidental affect, disposition, context, and retrospection, to derive propositions. Our DIA model can help managers gain a comprehensive understanding of how affect, characterized by its locus (self or other orientation) and valence (enhancing or harming), determines the characteristics of UEB. This essay gives an account of thee exchanges between Jacques Derrida and Hans-Georg Gadamer at the Goethe Institute in Paris in April 1981. Many commentators perceive of this encounter as an "improbable debate," citing Derrida's marginalization, or, in deconstructive terms, deconcentration of Gadamer's opening text as the main reason for its "improbabliity." An analysis of the questions that Derrida poses concerning "communication" as an axiom from which we derive decidable truth brings us to the central feature of this discussion: How (...) does one engage the "other" in conversation in the light of the problems reptaining to meaningful communication? The essay suggests that the first round of exchanges between Derrida and Gadamer is a good example of the violence that is prevalent (and perhaps inevitable) in all academic discussions. Finally a more "ethical" approach to discussion, based on Derrida's postulation of a friendship," is suggested. It challenges the hermeneutic search for consensus, whereby the "other" is contracted into fraternity, but cannot eliminate elements of violence completely. (shrink)

In this article we expand upon the conceptual framework of Félix Guattari’s 1989 essay, The Three Ecologies. Here Guattari examines changes in subjectivity that have come about due to scientific and technological advances which, as he sees it, brought about an “ecological disequilibrium” ([1989]2000, 27) and deteriorated individual and collective modes of being. In response to this Guattari proposes a kind of holistic therapy or ‘ecosophy’ between three ecological registers: “the environment, social relations and human subjectivity” (ibid., 28). Guattari cautions (...) that we not only take note of these macro determinations, but also heed the libidinal impulses of desire – what captures it, what directs it, and what shuts it down. Since the publication of The Three Ecologies, the rise of digital technologies has dramatically influenced the ways in which people think, live and communicate. Technological infrastructures, for instance, are used to spread disinformation at a vast speed and scale, and because social networks are often homophile many people infrequently encounter critical disagreement and debate. The increase in media and digital communications usage has also led to new forms of social inclusion and exclusion, the most exacerbated form of which is probably the phenomenon known as hikikomori – acute, prolonged social withdrawal. Taking hikikomori as our focus, we describe what we see as a general set of symptoms of society and trace its myriad intersections with an ever-increasing algorithmic ubiquity, which we posit as the new form of hyper-modulation following from Foucault’s disciplinary societies and Deleuze’s control societies. When Gilles Deleuze, in his book on Michel Foucault, asks, ‘who would think of looking for life among the archives?’, he uncovers something particular to Foucault's philosophy, but also to his own: a commitment to the question of what it means to think, and think politically. Although Foucault and Deleuze, who first met in 1952, immediately felt fondness for each other, a growing animosity had settled into the friendship by the end of the 1970s – a rift deepened by theoretical (...) differences. Notwithstanding these difficulties, Foucault and Deleuze shared a love for Nietzsche, as well as a curious fascination with Kant. Kant's influence, which was met with more opposition, is precisely the tension I tease out in this article, showing how Foucault critiques Kant's a priori through his own concept of the historical a priori, along with regularity and resistance, while Deleuze and Guattari, if more obliquely, critique Kant's a priori by further developing Foucault's notion of the historical a priori through their own method of pragmatics, particularly in three chapters of A Thousand Plateaus, namely ‘The Geology of Morals’, ‘Postulates of Linguistics’ and ‘On Several Regimes of Signs’. What I am interested in, then, is Deleuze and Guattari's treatment of redundancy, a concept mentioned seventy times in A Thousand Plateaus. Although not one of their main or most developed concepts, redundancy is a thread that can be traced back to Deleuze's first book, Empiricism and Subjectivity. Moreover, their more developed concepts, for example the diagram, the abstract machine, becoming, micro-politics and the ritornello, as well as their political philosophy, are grounded in their understanding of redundancy, which moves their philosophy beyond mapping out the conditions of possibility and demonstrating that the historical a priori emerges as a contingent function of its own expression to the genesis of thought. Kojève’s lectures on Phenomenology of Spirit generated two ideas – otherness is something threatening that must be overcome and one’s relationships with others are inexorably violent – that fundamentally shaped the way many exponents of early French phenomenology regarded intersubjectivity. This essay shows how Beauvoir’s appropriation of Hegel in The Ethics of Ambiguity offers a perspective on intersubjectivity that defies the other-conquering Cartesian hero implied by Kojève and celebrated in Sartre’s Being and Nothingness. Beauvoir appreciates the degree to which Hegel (...) makes subjectivity indebted to otherness. Like Hegel, she defines oppression as the failure to recognise the otherness of the other. Beauvoir shares Hegel’s optimism that individuals can sublate their naïve solipsism. She associates reciprocal recognition between subjects with ethical freedom, which she distinguishes from Sartre’s concept of freedom. From the analysis of ethical freedom it is concluded that both conflict and friendship are side-effects of the essential bond between subjects and their mutual need for one another. The Hegel-inspired hopefulness at the core of The Ethics of Ambiguity is further demonstrated by Beauvoir’s rejection of the absurd in favour of ambiguity, her positive rendering of failure, her appeal to outrageousness, and focus on the joy of existence.
